Reset Form - cannot submit as page is not functioning

I'm trying to submit a Commercial Use reset form, but cannot complete the process because of problems with the page. I am using a Win11 machine with the latest updates applied to all broswers:

Using Firefox, I can fill out the form and sign it, but the submit button doesn't do anything at all. I can click it and it flashes, but the screen does not change.

Using Edge and Chrome, I can fill out the form, but the signature box does not work - there is no way to draw in the box. Clicking submit just scrolls the page to the top, but does not seem to submit.

Something is broken?
