I am unable to reactivate my free licence. ID in wrong format

I have been directed to the personal use verification form and tried to fill it in. However the device ID section is telling me that my ID is in the wrong format?
Can anyone suggest what I am doing wrong please?

Please try typing the ID without spaces in between.
