How do I unblock my account

I went in to the unblock filled in all the information and then downloaded it. I forwarded it to my email address do I could have a copy and print it as a pdf, but now I can’t get to that message to print it out and send it in.

Is there a way to get this unblocked so I can continue to aid a non profit I volunteer for? It is a home to home network not business at all. It is a monastery with 9 monks. I work on their payroll and accounts payable for them free of charge and have been doing so since last year when the person who was helping them became deathly afraid of getting COVID and quit.
