I'd like to know if there is a service that should be running. How do I verify that realtime is running?

The only way to be sure is to check the logs or download the eicar test file.
The logs are located in: C:\Program Files\ITbrain\AntiMalware\ITbrainAntiMalware_Logfile.log
Eicar test file: https://www.eicar.org/?page_id=3950 download and run a file.
