Two way factor not working any more

I was unable to login using authenticator provided code. I've removed the two way using recovery code and was able to login. I'm now trying to activate two way again but I'm unable. After I scan the QR I'm asked to input code from the token which is not accepted. I've tryed multiple times... 

Any suggestions?

In my case I had a problem with correct time. My phone was not on auto sync time with the network. So, consequently phone was 45-50 seconds off and was giving out of sync codes...
